Biometric Screening & Wellness Credit
(Excludes Retiree Health Plan)
The biometric screening and flu shot period is September 1, 2021 – October 31, 2021
You and your enrolled spouse. Your enrolled spouse must be enrolled in full medical benefits to be eligible for the wellness program
Get a biometric screening (or enroll in the HealthyLife® Coaching Program), and receive a flu shot.
Complete the screening and receive your flu shot between September 1, 2021 and October 31, 2021.
At your doctor’s office. Note: blood work done by your doctor on or after March 1, 2021 will be accepted. All screenings must be completed and submitted by October 31, 2021.
OR At a participating local Kroger Pharmacy. Call 1-877-444-9689 or visit www.krogerscreenings.com to make your appointment. If you are screened at a Kroger Pharmacy, your results will be electronically submitted.
Reasonable Alternative: HealthyLife® Coaching Program
If you do not meet at least 2 out of the 4 health measures or do not participate in the biometric screening, you have the option to enroll in the HealthyLife® Coaching Program.
- Enroll in the American Institute for Preventive Medicine HealthyLife® Coaching Program HERE or by calling 1-800-345-2476 between September 1, 2021 and December 1, 2021; AND
- Successfully complete 4 coaching calls by March 1, 2022.
Biometric Screening or HealthyLife® Incentive: $150 Wellness Incentive
You and your spouse (if enrolled) will each receive a $150 wellness incentive for completing the Biometric Screening Form by October 31, 2021, and meeting at least 2 out of the 4 health measures OR successfully completing the HealthyLife® Coaching Program requirements.
Flu Shot Incentive: $50 Wellness Incentive
If you and your spouse (if enrolled) each receive a flu shot by October 31, 2021, you will each receive an additional $50 wellness incentive. The flu shot does not impact your co-premiums.
You do not have to submit a form indicating that you received your flu shot, unless you plan on receiving it from a place where your insurance will not be billed. In this case, you will need to complete and fax a Flu Shot Form to the Plan Administrative Office by October 31, 2021.
Kroger Employees: The $50 or $150 incentive is a reimbursement. Once you spend this amount of money out of your pocket toward your 2022 yearly deductible, you will receive your wellness reimbursement check in the mail. Administration of the Wellness Credit
Non-Kroger Employees: The $50 or $150 is an incentive that can be used to offset your deductible and required coinsurance for the 2022 calendar year.
Weekly Co-premium Incentive
Additionally, you can pay a lower weekly co-premium ($9 less per week) by meeting the biometric screening requirements OR successfully completing the HealthyLife® Coaching Program. Check the co-premium chart in your collective bargaining agreement for more details.
Unsuccessful Completion of Wellness Program
- If you do not complete and meet the biometric screening requirements by October 31, 2021; OR
- If you do not meet at least 2 out of the 4 health measures AND do not enroll in the HealthyLife® Coaching Program AND complete 4 coaching calls by March 1, 2022.
Then you will not be eligible to receive the lower weekly co-premium incentive and you will not earn the $150 wellness incentive.
If you have a medical condition that prevents you from successfully completing the biometric screening or receiving the flu shot, complete and fax a Medical Waiver to the Plan Administrative Office by October 31, 2021. Waivers submitted after the deadline will not be accepted.
Participation in the biometric screening does not guarantee eligibility for benefits. In general, your eligibility for the Plan and level of benefits you may receive under the Plan are based upon your employer contributing to the Plan on your behalf.
